Knee Injection Options: How Doctors Choose Between Them

Knee injection options include corticosteroid, hyaluronic acid and blood-derived preparations such as PRP or GFC, but they are not equivalent and are not suitable for every painful knee. Selection begins with diagnosis, arthritis stage, symptom pattern, previous treatment, medical risk and the goal of the injection.

An injection may modify symptoms for a period; it should not be described as reliably regrowing cartilage, reversing established arthritis or guaranteeing avoidance of surgery.

Knee Injection Options Should Follow a Diagnosis

Knee pain may come from osteoarthritis, inflammatory disease, injury, infection, referred pain or another cause. Injecting a knee without understanding the likely diagnosis can delay appropriate treatment. Examination and suitable imaging help determine whether the target is the joint and whether an injection could change function.

Corticosteroid Injection

A corticosteroid injection may offer short-term relief for selected people, particularly when pain or inflammation prevents participation in exercise and other medicines are ineffective or unsuitable. Repeated or poorly timed injections may carry risks, and temporary symptom relief does not change the underlying arthritis.

Hyaluronic Acid Injection

Hyaluronic acid is marketed as a lubricant injection. Recommendations differ between settings, and NICE advises not offering intra-articular hyaluronan for osteoarthritis. A patient should understand the guideline position, cost, uncertainty and alternatives before considering it.

PRP and GFC

Platelet-rich plasma and growth-factor concentrate are prepared from the patient’s blood using different processing methods. Evidence, preparation protocols and response vary. NICE states that PRP for knee osteoarthritis raises no major safety concern but that efficacy evidence is limited in quality.

  • Confirmed diagnosis and the painful compartment

  • Mild, moderate or advanced structural disease

  • Swelling or inflammatory features

  • Previous response and duration of relief

  • Diabetes, blood-thinner use, infection risk and allergies

  • Upcoming surgery and timing considerations

  • Ability to combine symptom relief with rehabilitation

  • Cost, evidence certainty and patient preference

When an Injection Is Less Likely to Solve the Problem

  • The pain source is outside the knee

  • There is suspected infection or an unsafe skin condition

  • Advanced disabling arthritis has already failed appropriate non-surgical care

  • Mechanical instability or deformity is the main limitation

  • The proposed benefit is based on cartilage-regrowth promises

  • Repeated injections have produced little meaningful functional benefit

Risks and Aftercare Questions

Risks vary by product and patient. They can include pain flare, bleeding, infection, allergic reaction and temporary blood-sugar elevation after steroid. Ask what activity is advised, which medicines to continue, when benefit should be assessed and which symptoms require urgent review.
