GFC Injection Side Effects, Aftercare and Warning Signs

GFC injections use a patient’s own blood to prepare a concentrated growth-factor-containing product for injection into the knee. Temporary soreness or swelling can occur after the procedure. Serious complications are uncommon when appropriate sterile technique and patient selection are used, but infection, bleeding and other problems remain possible.

The exact frequency of adverse effects cannot be stated reliably for every GFC preparation because preparation systems, platelet and leukocyte characteristics, injection protocols and reporting methods vary.
What Does GFC Mean?
GFC is generally used to describe a growth factor concentrate prepared from autologous blood. Blood is collected, processed using a proprietary or laboratory-specific system, and the resulting product is injected into the affected joint.
GFC is related to the broader family of platelet-derived orthobiologic preparations, but the terms GFC and PRP should not be assumed to describe identical products. Concentrations, cell content, activation method, volume and dosing protocol may differ.

Common Short-Term Reactions
Injection-Site Pain
Needle insertion can cause local soreness. Some patients also experience a transient inflammatory flare inside the knee, with increased discomfort during walking or bending. This usually settles, but the treating clinician should advise what duration and severity are expected in the individual case.
Temporary Swelling or Stiffness
A small increase in swelling or stiffness may occur after an intra-articular injection. Activity should be adjusted according to the treating team’s instructions. Rapidly increasing swelling, severe pain or inability to bear weight deserves assessment rather than being assumed to be a routine reaction.
Bruising
Bruising can occur at the blood-draw site or around the injection. The risk may be greater in patients taking antiplatelet or anticoagulant medication or those with a bleeding disorder. Prescribed medicines should never be stopped without instructions from the clinician responsible for them.
Important but Less Common Risks
Joint Infection
Infection after a joint injection is uncommon but potentially serious. Sterile preparation and injection technique reduce risk but cannot make it zero. Increasing warmth, redness, severe pain, fever, wound discharge or rapidly worsening swelling needs prompt medical review.
Bleeding Into or Around the Joint
Bleeding is possible, particularly when clotting is impaired. The injection clinician should know about anticoagulants, antiplatelet agents, liver disease, low platelet counts, previous bleeding problems and all current medicines.
Injury to Nearby Structures
Pain, bleeding or irritation can occur if a needle affects a nearby vessel, nerve, tendon or other structure. The risk depends on anatomy, the injection site and technique.
Treatment Failure or Delayed Appropriate Care
The most important practical limitation is that the injection may not provide meaningful benefit. Temporary symptom relief also does not prove that cartilage has regenerated. Repeated injections should not delay reassessment when arthritis, deformity or disability is progressing.
When to Seek Urgent Review
Fever or chills after the injection
Rapidly increasing knee warmth, redness or swelling
Severe or escalating pain rather than a mild temporary flare
New inability to bear weight
Persistent wound discharge or bleeding
New calf swelling, chest pain or breathlessness
A widespread allergic-type reaction or breathing difficulty
Chest pain, breathlessness or severe systemic symptoms require urgent emergency assessment. This list is not exhaustive; patients should use the contact instructions supplied by their treating clinic.
Aftercare Following a GFC Injection
Aftercare protocols vary. The treating clinician’s instructions take priority because they account for the injection method, medicines, other diagnoses and the patient’s health.
General measures may include:
Relative reduction of strenuous activity for the period advised by the treating team
Continuing ordinary gentle movement within comfort unless told otherwise
Using a covered cold pack for short periods if the clinician recommends it
Taking only pain medication that has been approved for the individual patient
Avoiding self-directed changes to anticoagulants, antiplatelet medicines or anti-inflammatory drugs
Recording pain, swelling and function rather than judging the result from one day
Attending the planned review and rehabilitation programme
There is no universally accepted post-injection protocol. Claims that one specific restriction or supplement guarantees a better biologic response are not established.
Who Needs Additional Caution?
Suitability requires medical assessment. Additional caution may be needed in patients with active infection, skin infection near the proposed injection site, significant bleeding risk, blood disorders, poorly controlled systemic illness, immunosuppression, pregnancy-related considerations or an uncertain diagnosis.
A painful swollen knee should not automatically receive a biologic injection. Depending on the presentation, fluid analysis or investigation for infection, crystal arthritis or inflammatory disease may be more important.
When Might GFC Be Considered?
A blood-derived injection may be discussed for selected patients with symptomatic knee osteoarthritis when diagnosis, arthritis stage, alignment, symptoms, previous treatment and expectations have been assessed. It is not automatically appropriate for every painful knee.
Evidence for platelet-derived injections suggests that some patients may report symptom improvement, but study methods and products are heterogeneous. Evidence does not establish reliable cartilage regrowth or reversal of advanced osteoarthritis.
When Is It Less Likely to Help?
An injection is less likely to solve the central problem when there is advanced structural destruction, major deformity, substantial instability, severe fixed stiffness, another untreated diagnosis or disability that is no longer adequately managed with non-surgical treatment.
Patients with severe symptoms can review when knee arthritis may need replacement rather than assuming that further injections are always the next step.
