When Should Back, Neck or Sciatica Symptoms Be Assessed?
Back and neck pain are common and often improve with time, movement and appropriate conservative care. An orthopedic assessment becomes more useful when pain persists, repeatedly returns, limits work or daily activity, follows an injury, or is accompanied by symptoms in an arm or leg.

What Is Sciatica?
Sciatica describes pain or neurological symptoms that usually travel from the lower back or buttock into the leg. Tingling, numbness or weakness may occur. Not every pain travelling down the leg is sciatica, and a scan finding such as a disc bulge does not prove that it is the pain source.
Symptoms That Merit a Planned Review
Pain that persists or repeatedly interferes with daily activity
Pain extending into an arm or leg
Recurring numbness, tingling or weakness
Pain following a fall or other injury
Progressive loss of movement or difficulty walking normally
Prolonged morning stiffness or symptoms affecting several joints
When Urgent Assessment Is Appropriate
New severe neurological symptoms, rapidly worsening weakness or sensation, major trauma, fever with severe spinal pain, or other serious warning features require urgent hospital or specialist assessment rather than a routine clinic appointment.
How the Initial Assessment Is Performed
The history should identify where the pain starts, whether it travels, which movements or positions change it, whether sleep or walking is affected, and whether numbness or weakness is present. Previous injuries, osteoporosis risk, inflammatory symptoms, medical history and current medicines may change the investigation or referral plan.
The examination may assess walking, spinal and hip movement, strength, sensation, reflexes and circulation. Hip, knee or shoulder problems can sometimes mimic spinal pain, while spinal or nerve problems can present as limb pain. Clinical correlation is essential before attributing symptoms to an imaging finding.
When Are X-Rays or MRI Needed?
Imaging is not automatically required for every episode of back or neck pain. X-rays may help after injury or when alignment, fracture, arthritis or another bone problem is suspected. MRI is considered when neurological symptoms, warning features, persistent symptoms or a possible diagnosis make the result likely to change treatment or referral.
Disc bulges and age-related changes are common and may be present without symptoms. The report must be matched with the pain distribution and examination rather than treated in isolation.
What Non-Surgical Treatment May Involve
For many non-urgent mechanical problems, treatment may include education, remaining active within tolerance, temporary modification of aggravating activity, appropriate medication and a progressive rehabilitation programme. Exercise selection and progression should match symptoms, strength and neurological findings.
Persistent symptoms should prompt reassessment rather than automatic escalation to injections or surgery. The diagnosis, neurological status, rehabilitation quality and need for another specialist should be reviewed.
When Referral Is Appropriate
Referral to a spine surgeon, neurologist, rheumatologist, rehabilitation physician or pain specialist may be appropriate when the diagnosis, examination or required treatment falls outside general orthopedic care. Surgical referral is not based on MRI wording alone. It depends on symptoms, function, progression and response to suitable non-surgical treatment.
