Knee Pain From Back: Why a Normal Knee X-Ray Can Still Mean Severe Pain
Many patients in Mumbai come to Dr. Mayur Rabhadiya with knee pain even though their knee X-ray is completely normal. This confuses both patients and sometimes even doctors. The reason is that the pain is not coming from the knee joint. It is coming from the lower back.
The nerves that supply sensation to the knee originate from the lumbar spine. When these nerves are irritated due to a slip disc, disc bulge, or spinal degeneration, the brain feels pain in the knee even though the knee joint itself is healthy. This is called referred pain or lumbar radiculopathy and is a very common cause of unexplained knee pain.
How Sciatica and Lumbar Radiculopathy Cause Knee Pain
Sciatica occurs when a nerve in the lower spine gets compressed. This compression may happen due to a slipped disc, spinal narrowing, or age-related wear of the spine. The sciatic nerve and its branches travel from the lower back to the thigh, knee, and leg.
When this nerve is irritated, pain is not felt only in the back. It can be felt in the buttock, thigh, knee, or even the foot. This is why many patients with knee pain are treated with medicines, injections, or physiotherapy for the knee but get no relief, because the real problem is in the spine.
This pattern of pain is called lumbar radiculopathy, and it is one of the most commonly missed causes of chronic knee pain.
Symptoms That Suggest Your Knee Pain Is Coming From the Spine
If you have knee pain from back problems, you will often notice some of these signs:
• Knee pain but normal X-ray
• Pain that starts in the lower back and travels to the thigh or knee
• Burning, tingling, or electric shock type pain
• Pain that worsens on sitting, walking, or bending
• Weakness or numbness in the leg
These symptoms are typical of sciatica and lumbar nerve compression rather than knee arthritis.
Why Knee Treatments Fail When the Pain Comes From the Back
Many patients in Ghatkopar, Chembur, Kurla, Vidyavihar, Vikhroli, Mulund, Andheri, and Powai take knee medicines, get injections, or even undergo unnecessary knee procedures when the true cause is spinal.
Since the knee joint is not damaged, these treatments do not work. Relief only comes when the nerve compression in the spine is diagnosed and treated correctly. That is why an orthopedic and joint replacement surgeon trained in spine and nerve-related pain must evaluate such cases.
When You Should Get a Spine Evaluation Instead of Knee Treatment
You should consult a spine-trained orthopedic surgeon if:
• Your knee X-ray is normal but pain persists
• Pain radiates from back to knee
• You feel tingling, numbness, or burning
• Pain worsens on sitting or long walking
• Knee injections give little or no relief
These are classic signs of knee pain from back or sciatica.
