Normal Knee X-Ray Limitations: What It Can and Cannot Show

Normal knee X-ray limitations matter because radiographs show bones, alignment and indirect signs of joint-space loss, but they do not show every pain-sensitive structure clearly. A normal report does not mean the pain is imaginary, and it does not automatically mean an MRI is required.

Normal Knee X-Ray Limitations: What Radiographs Show Well
Fractures and many bone abnormalities
Joint alignment and deformity
Joint-space narrowing when images are taken appropriately
Osteophytes and subchondral bone changes
Some loose bodies or calcification
Changes after previous surgery or implants
Weight-bearing views can reveal narrowing and alignment under load that may be less apparent on a non-weight-bearing image. The view requested should match the clinical question.
What a Normal Knee X-Ray Does Not Exclude
Meniscal or ligament injury
Early cartilage or patellofemoral problems
Tendon and muscle disorders
Bone-marrow oedema or some stress injuries
Synovitis and smaller joint effusions
Pain referred from the hip, back or nerves
Intermittent inflammatory conditions
Common Reasons for Knee Pain With a Normal X-Ray
Pain may arise from kneecap overload, early osteoarthritis, tendon irritation, weakness, a meniscal problem, a stress response in bone or a source outside the knee. When MRI May Be Useful
MRI is more likely to help when pain persists after appropriate assessment, the initial radiographs are normal or do not explain symptoms, there is suspected internal derangement, locking, stress injury, osteonecrosis, infection, tumour or a treatment decision that genuinely depends on soft-tissue detail.

Swelling, warmth, tenderness and movement
Stability, gait and muscle strength
Hip and spine examination when indicated
Whether the original X-rays included suitable standing views

Inability to bear weight after trauma
A hot, swollen knee with fever
Rapidly increasing swelling or redness
True locking that blocks movement
Persistent night pain with systemic symptoms
New calf swelling, chest pain or breathlessness
