Knee MRI Findings: Why Pain Can Persist Even When MRI Looks Normal

Knee MRI findings should be interpreted alongside symptoms and examination because a scan can look reassuring while clinically meaningful pain is still present. Knee MRI findings are interpreted in context. A scan can show menisci, ligaments, cartilage, bone marrow, tendons, fluid and other soft tissues, but it cannot by itself prove which finding is causing pain. Doctors compare the report and images with the pain pattern, examination, weight-bearing X-rays and the clinical question that prompted the scan.

Treatment should target the patient’s problem, not every abnormal phrase in a radiology report.

Knee MRI Findings: Why Symptoms May Not Match the Scan

MRI is sensitive, so it commonly identifies age-related or incidental changes. A meniscal signal, small cartilage defect, mild fluid or degenerative change may be relevant, partly relevant or unrelated. The location and timing of pain help determine significance.

For example, front-of-knee pain on stairs may relate more to the kneecap joint and load tolerance than to a minor meniscal change. Conversely, focal bone-marrow oedema or a stress injury may explain substantial pain even when a plain X-ray appears unremarkable.

What a Knee MRI Can Show

  • Meniscal tears, degeneration or displacement

  • Anterior and posterior cruciate ligament injury

  • Collateral ligament and tendon abnormalities

  • Cartilage defects and osteoarthritis-related changes

  • Bone bruising, stress injury, cysts or marrow lesions

  • Joint fluid, synovitis and some loose bodies

  • Soft-tissue masses or other less common causes

Why an Abnormal MRI May Not Equal the Pain Source

Degenerative findings become more common with age, including in people without major symptoms. Doctors therefore ask whether the abnormality is on the painful side, whether examination reproduces the same pain, whether symptoms followed an injury, and whether the proposed treatment would change if the finding were absent.

Can MRI Be Normal Despite Real Knee Pain?

Yes. Pain may arise from early load-related change, patellofemoral overload, weakness, a pain source outside the knee, intermittent inflammation or a condition that is not well captured during the scan. A normal MRI should prevent unnecessary treatment of structures that look normal, but it should not be used to dismiss persistent symptoms.

Why Weight-Bearing X-Rays Still Matter

MRI is performed lying down. Standing X-rays can show joint-space loss, alignment and deformity under load, which may be central when osteoarthritis is suspected. Imaging choice depends on the question: radiographs are usually the initial study for chronic knee pain, while MRI becomes useful in selected situations.

When MRI Is More Likely to Change Management

  • Persistent pain after appropriate initial assessment with normal or non-explanatory X-rays

  • Suspected internal derangement after injury

  • Locking or a true mechanical block

  • Concern for stress injury, osteonecrosis, tumour or infection

  • Planning treatment when the diagnosis remains uncertain

  • Symptoms that do not fit the apparent severity or location of arthritis

  • Which finding best matches my symptoms and examination?

  • Is the abnormality common in people without symptoms?

  • Would treatment change without this MRI finding?

  • Do I also need standing X-rays or another test?

  • What non-surgical step should be tried first?

  • Which warning signs should prompt earlier review?
