Burning Knee Pain
Burning knee pain may feel hot, stinging, tingling, electric, raw or unusually sensitive to clothing and touch. The sensation may be deep within a swollen joint, localised to a tendon or bursa, superficial in the skin, distributed along a nerve or referred from the lower back. “Burning” describes the quality of pain; it does not identify one diagnosis.
The associated features matter. Warmth, swelling and stiffness suggest inflammation. Numbness, tingling, electric shocks or weakness suggest nerve involvement. A rash or spreading redness suggests a skin condition. Pain travelling from the back or thigh may be referred. Persistent burning should therefore be assessed rather than treated only with creams or repeated knee injections.

Quick Answer: What Causes a Burning Sensation in the Knee?
Common causes include an osteoarthritis flare, joint swelling, patellofemoral irritation, tendon or bursal inflammation, local nerve irritation and pain referred from the lumbar spine. Burning near a surgical scar can result from healing sensory nerves, while diabetes and other peripheral neuropathies may affect sensation in both legs.
Urgent causes include a hot red swollen joint with infection or crystal arthritis, rapidly spreading skin infection, acute neurological weakness, a cold discoloured foot, severe postoperative deterioration or symptoms of a blood clot. The location, onset and associated signs determine urgency.
Where Is the Burning Felt?
Burning Around or Behind the Kneecap
Burning or aching around the kneecap may occur with patellofemoral pain, patellofemoral arthritis or an activity-related flare. It may be worse on stairs, during squatting, after prolonged sitting or while getting up from a chair. See Front Knee Pain.
Burning Below or Above the Kneecap
Localised burning below the patella may involve the patellar tendon, infrapatellar fat pad, bursa or a superficial sensory nerve. Burning above the kneecap may come from quadriceps-tendon overload. Jumping, running, hills, heavy squats and sudden exercise changes are common triggers.
Burning on the Inner Side
Inner-side burning may arise from medial-compartment arthritis, pes anserine tendon or bursal irritation, medial ligament sensitivity or the saphenous nerve and its infrapatellar branch. Pain that is very superficial, sensitive to light touch or extends along a narrow strip is more suggestive of a sensory-nerve component. See Inner Knee Pain.
Burning on the Outer Side or Near the Fibular Head
Outer burning may occur with iliotibial-band-region pain, lateral arthritis, tendon irritation or peroneal-nerve irritation near the fibular head. Numbness over the outer leg, weakness lifting the foot or foot drop requires prompt neurological evaluation. See Outer Knee Pain.
Burning Behind the Knee or Into the Calf
Posterior burning may come from a Baker’s cyst, calf or hamstring strain, nerve referral or vascular disease. Sudden calf swelling, warmth, colour change, chest pain or breathlessness requires urgent assessment for deep-vein thrombosis. See Pain Behind the Knee.
Burning That Travels From the Back or Thigh
Pain travelling from the lower back, buttock or thigh toward the knee may reflect lumbar nerve-root irritation or spinal stenosis. Numbness, tingling, altered reflexes or weakness strengthen this possibility. Read Knee Pain From the Back or Spine.
Common Causes of Burning Knee Pain
Knee Osteoarthritis Flare
An osteoarthritis flare can cause deep aching, warmth, swelling and stiffness after walking, stairs or increased activity. Typical osteoarthritis symptoms include brief stiffness after rest, reduced walking distance, crepitus and progressive deformity. A severely hot red knee is not typical of a routine flare and needs assessment for gout, infection or another acute inflammatory condition.

Joint Inflammation, Gout or Infection
Gout and other crystal arthritis can cause sudden intense burning, swelling, redness and extreme tenderness. Infection can look similar and may occur with fever or systemic illness. A hot swollen joint may require urgent aspiration and laboratory testing rather than routine home treatment.
Tendon, Bursa or Fat-Pad Irritation
Tendons and bursae can produce a local burning or tender sensation that is reproduced by pressure or a specific movement. Common locations include the patellar tendon, quadriceps tendon, pes anserine region and prepatellar bursa. Treatment generally uses activity-load adjustment and progressive rehabilitation rather than an intra-articular arthritis injection.
Local Sensory-Nerve Irritation
Small sensory nerves around the knee may become irritated after trauma, prolonged kneeling, compression or surgery. The skin may be hypersensitive, numb or painful to light touch. A focal neuroma can occasionally develop near a scar. Diagnosis depends on the sensory distribution, examination and relationship to injury or operation.
Lumbar Nerve-Root or Peripheral-Nerve Pain
Lumbar disc or spinal changes can irritate nerves that supply the thigh and knee region. Peripheral nerves such as the femoral, saphenous or peroneal nerves can also be involved. Burning with numbness, tingling, altered reflexes or weakness is more neuropathic than ordinary joint pain. The treatment target may therefore be the spine or nerve rather than the knee joint.
Peripheral Neuropathy
Diabetes, vitamin deficiency, thyroid disease, alcohol-related neuropathy, selected medicines and other systemic conditions can alter sensation. Symptoms are often bilateral and extend into the feet, although patterns vary. Medical evaluation may include neurological examination and targeted blood tests.
Skin Infection, Shingles or Dermatological Causes
Cellulitis can cause spreading redness, warmth, tenderness and fever. Shingles may begin with burning or skin sensitivity before a one-sided blistering rash appears. Contact dermatitis, insect bites and other skin conditions can also burn. The skin should be inspected rather than assuming every burning sensation comes from inside the knee.
Pain After Knee Surgery or Replacement
Temporary numbness or burning near an incision can occur as sensory nerves heal. Persistent severe hypersensitivity, colour or temperature change, marked swelling, weakness, wound drainage, fever or progressive pain needs review. Rarely, complex regional pain syndrome can cause disproportionate burning, swelling, colour change and sensitivity and requires early multidisciplinary assessment.
Vascular Causes
Reduced arterial circulation can cause exercise-related calf pain, a cold foot, colour change or non-healing wounds. A blood clot can cause swelling and warmth. A cold pale or blue foot, absent pulses, sudden calf swelling, chest pain or breathlessness requires urgent medical assessment.
How Burning Knee Pain Is Evaluated
The consultation should establish whether the sensation is deep or superficial, the precise distribution, onset, activity relationship, skin changes, swelling, back pain, numbness, weakness, diabetes, previous injury or surgery and effect on walking and sleep. Examination may include the knee, skin, scars, tendons, temperature, pulses, sensation, strength, reflexes, hip and spine.
Do I Need X-Rays, MRI, Nerve Tests or Blood Tests?
Standing X-rays may help when osteoarthritis or a bone problem is suspected. MRI is selected for a specific joint, soft-tissue, bone or spinal question when the result will change management. Nerve-conduction studies may be useful for selected peripheral-nerve or neuropathy patterns but do not diagnose every pain syndrome.
Blood tests may be considered for infection, inflammation, gout, diabetes, vitamin deficiency, thyroid disease or other systemic causes. Ultrasound or vascular imaging may be needed for swelling, a suspected clot or circulation problem. Normal knee imaging does not exclude nerve or referred pain.
Treatment for Burning Knee Pain
Treat the Diagnosed Source
Joint inflammation, a tendon problem, lumbar nerve pain, neuropathy and skin infection require different treatment. A knee injection is inappropriate when the main source is a nerve, skin, blood vessel or spine. Treatment may involve arthritis care, progressive rehabilitation, neurological treatment, correction of a systemic cause, antimicrobial treatment or urgent vascular management.
Activity Modification and Rehabilitation
For non-urgent mechanical causes, temporarily reduce the movement or load that reliably provokes symptoms while preserving safe activity. A programme may include quadriceps and hip strengthening, tendon loading, mobility and gait retraining. Neuropathic symptoms may require desensitisation, neural assessment and treatment of the originating nerve or spine.
Cold, Warmth and Skin Care
Cold may help a warm swollen arthritic flare, while gentle warmth may help stiffness without active heat or redness. Extreme temperature can injure skin with reduced sensation. Do not place heat over spreading redness, a suspected infection or a numb area, and do not sleep with a heat or ice device in place.
Medicines and Injections
Medicine selection depends on whether pain is inflammatory, mechanical or neuropathic and on kidney, stomach, heart, liver and medication risks. Neuropathic medicines are not ordinary painkillers and require medical supervision. Corticosteroid injection may provide short-term relief for selected osteoarthritis patients but does not treat spinal nerve pain, neuropathy, infection or skin disease. Evidence and recommendations vary for hyaluronic acid, PRP, GFC and other injections, and none should be described as guaranteed cartilage regeneration.
When Is Surgery or Knee Replacement Considered?
Burning pain alone is not an indication for knee surgery. Surgery is considered only when a clearly defined structural problem matches the symptoms and appropriate non-surgical treatment is ineffective or unsuitable. Nerve-related, cutaneous or referred pain may persist after unnecessary knee surgery, which is why identifying the source is essential.
Knee replacement may be considered when confirmed advanced arthritis substantially affects walking, stairs, sleep, deformity and independence despite suitable care. Robotic assistance supports planning and execution but does not replace surgical judgement. A hot, red, rapidly swollen and severely painful knee

Questions Patients Commonly Ask
Does burning knee pain always come from a nerve?
No. Arthritis, inflammation, tendon or bursal pain, skin disease and vascular conditions can also feel burning.
How can I tell nerve pain from arthritis pain?
Numbness, tingling, electric shocks, skin sensitivity and weakness suggest nerve involvement. Arthritis more commonly causes activity pain, stiffness, swelling and reduced movement. Overlap is possible.
Can a back problem cause burning around the knee?
Yes. Lumbar nerve irritation can refer burning, tingling or weakness toward the knee even when knee imaging is normal.
Can diabetes cause burning knees?
Diabetic neuropathy can alter sensation, usually in the feet and lower legs and often on both sides. A focal knee symptom still requires assessment for local causes.
Why does my skin hurt when clothes touch the knee?
Allodynia or hypersensitivity can occur with sensory-nerve irritation, a healing scar, shingles or complex regional pain. Skin and neurological examination are important.
Can shingles begin as burning knee pain?
Yes. Burning or skin sensitivity may precede a one-sided blistering rash. Prompt medical assessment is appropriate because antiviral treatment is time-sensitive.
Is warmth the same as burning nerve pain?
No. A physically warm swollen joint suggests inflammation, while neuropathic burning may occur without objective heat. Examination distinguishes the patterns.
Why does my knee burn after walking or exercise?
An arthritic flare, patellofemoral overload, tendon irritation, training-load increase or nerve sensitivity may contribute. Location and next-day response help guide diagnosis.
Is burning near a knee-replacement scar normal?
Temporary numbness and burning can occur as superficial nerves heal. Increasing redness, drainage, fever, marked swelling or worsening pain needs urgent review.
Can complex regional pain syndrome affect the knee?
It can occur after injury or surgery and may cause disproportionate burning, sensitivity, swelling, temperature or colour change and stiffness. Early specialist assessment is important.
Can an X-ray identify the cause of burning pain?
It can show arthritis or bone abnormalities but cannot assess every nerve, skin, tendon or spinal cause. Testing should follow examination.
Do I need a nerve-conduction test?
Only selected patterns require it. The test is most useful when peripheral-nerve injury or neuropathy is suspected and the result would change treatment.
Should I use heat or ice?
Cold may help a warm swollen flare; warmth may help stiffness. Avoid extreme temperature on numb skin or over suspected infection.
Will a knee injection help burning pain?
Only when a suitable diagnosed joint condition is the source. It will not treat lumbar nerve pain, peripheral neuropathy, shingles, skin infection or vascular disease.
Does burning pain mean I need knee replacement?
No. Replacement is considered only when advanced confirmed arthritis substantially affects overall function and quality of life despite suitable care.
