Persistent back pain should be reassessed when it repeatedly limits work, sleep, walking or daily activity, or when symptoms extend into a limb. The cause cannot be determined from pain duration alone.

Persistent Back Pain: Causes, Assessment and Referral
Common causes include muscular or joint-related pain, disc and nerve irritation, reduced conditioning or pain referred from another area. Imaging is not routine for every patient and should be selected when it is likely to change management.
Urgent assessment is needed for new bladder or bowel disturbance, saddle numbness, progressive weakness, fever, major trauma, unexplained weight loss or severe unremitting symptoms. Dedicated spine referral may be required after initial assessment.
