Blood Clots After Knee Replacement
Blood clots are an important safety concern after knee replacement because major lower-limb surgery, temporary reduced mobility and the body’s normal clotting response can increase the risk of venous thromboembolism. Venous thromboembolism includes deep-vein thrombosis in the leg and pulmonary embolism when part of a clot travels to the lungs.
Most patients do not develop a clinically important clot, and preventive measures reduce risk. However, symptoms can overlap with normal postoperative pain and swelling, and some clots produce few or no symptoms. Prevention, medicine adherence and recognition of emergency signs are therefore all important.
Quick Answer: What Are the Warning Signs?
Possible DVT symptoms include new or increasing calf pain, tenderness, warmth, redness or unequal swelling of the calf, ankle or foot. Possible pulmonary-embolism symptoms include sudden breathlessness, chest pain, coughing blood, a rapid or irregular heartbeat, light-headedness, fainting or collapse. Pulmonary-embolism symptoms require immediate emergency care.
Why Knee Replacement Increases Clot Risk
Surgery activates clotting mechanisms and affects veins in the operated limb. Pain, anaesthesia, weakness and fatigue can reduce walking during the first days. Risk continues after discharge because mobility and muscle pumping remain below normal. This is why the prevention plan often continues at home and why discharge does not mean that clot risk has ended.
Individual Factors That Increase Risk
Risk can increase with a previous DVT or pulmonary embolism, inherited clotting disorder, strong family history, cancer, obesity, older age, major immobility, hormone therapy, pregnancy-related factors, heart disease, inflammatory disease and multiple medical conditions. Bilateral or revision surgery may add physiological and mobility demands. Several moderate factors together can be more important than one factor alone.
Bleeding Risk Also Matters
A stronger anticoagulant strategy can reduce clot risk but may increase wound bleeding, bruising, haematoma and other bleeding. The plan therefore balances clot risk against bleeding risk, kidney function, liver disease, platelet problems, other medicines, anaesthesia and the operation performed. The best prevention medicine is not identical for every patient.
Medicines Used for Prevention
Depending on individual risk and local protocol, prevention may use aspirin, low-molecular-weight heparin, a direct oral anticoagulant or another anticoagulant. The exact drug, start time, dose and duration must be prescribed by the treating team. Patients should not choose a medicine based on another person’s prescription or add aspirin to an existing anticoagulant without advice.
Early Mobilisation and Ankle Exercises
Standing and walking when medically safe activate the calf muscles and reduce prolonged venous stasis. Ankle pumps and regular position changes also help circulation. These measures support, but do not replace, prescribed medication in patients who require it. Complete bed rest without a medical reason can increase clot, stiffness and weakness risk.

Compression Devices and Stockings
Intermittent pneumatic compression devices may be used in hospital, and graduated compression stockings may be prescribed for selected patients. Fit matters. Rolled, folded or excessively tight stockings can injure skin or impair comfort. Compression should not be self-prescribed when arterial circulation, wounds, severe swelling or skin disease may make it unsuitable.
Normal Postoperative Swelling Versus Possible DVT
Normal swelling can involve the knee, calf, ankle and foot and often worsens when the leg remains down or after activity. It may improve with elevation and time. A clot is more concerning when swelling is new, rapidly increasing, clearly unequal, associated with calf tenderness, warmth or redness, or does not improve with elevation. Symptoms alone cannot reliably exclude or confirm DVT.

Can a DVT Occur Without Symptoms?
Yes. CDC notes that many people with DVT have no symptoms. This is one reason preventive measures are applied based on risk rather than waiting for pain or swelling. Routine screening of every patient is not automatically required; testing is guided by symptoms, examination and clinical risk.
How DVT Is Diagnosed
Duplex ultrasound is commonly used to examine the deep veins of the leg. The doctor also reviews surgery timing, symptoms, medicine adherence and risk factors. Blood tests such as D-dimer are often less specific soon after surgery because normal healing can raise the result. When clinical concern remains high, further assessment or repeat imaging may be needed.
How Pulmonary Embolism Is Diagnosed
Pulmonary embolism requires urgent hospital evaluation. Assessment may include oxygen level, electrocardiogram, blood tests and specialised chest imaging. A patient can have pulmonary embolism without obvious calf symptoms. Sudden breathlessness or chest pain after surgery should not be observed at home or attributed only to anxiety.
Treatment of a Confirmed Clot
Confirmed DVT or pulmonary embolism is commonly treated with therapeutic anticoagulation to prevent clot extension and further embolism. Medicine choice and duration depend on clot location, symptoms, kidney function, bleeding risk, interactions and whether pulmonary embolism is present. Unstable pulmonary embolism can require intensive hospital treatment and specialised procedures.
Anticoagulant Safety and Bleeding Warning Signs
Anticoagulants can increase bruising and bleeding. Contact the treating team for repeated wound bleeding, expanding haematoma, blood in urine, black stool, prolonged nosebleed or unusual weakness. Vomiting blood, severe headache after a fall, uncontrolled bleeding, collapse or new neurological symptoms requires emergency assessment. All treating doctors and dentists should know that anticoagulants are being used.
Missed Doses and Medicine Changes
Do not double a missed anticoagulant dose unless the prescribed instructions specifically say to do so. Do not stop medicine because of ordinary bruising without contacting the treating team. Over-the-counter anti-inflammatory medicines, aspirin, herbal products and supplements can affect bleeding or interact with anticoagulants and should be reviewed before use.
Car Travel and Prolonged Sitting
Long car journeys combine immobility, dependent leg position and limited opportunity to elevate. When travel is approved, plan regular stops for short walks, ankle movements and position changes. Keep medicines accessible and take them on schedule. Travel should be postponed when the wound is draining, the leg is markedly swollen or medical review is pending.
Flying After Knee Replacement
Recent surgery and long-distance travel can combine to increase clot risk. Flight timing, airport mobility, previous clot history, wound condition, anticoagulant plan and total door-to-door travel time should be reviewed. Do not independently take an extra dose, add aspirin or extend anticoagulation for a flight.

Partial, Robotic, Bilateral and Revision Surgery
Partial replacement may permit earlier mobility in selected patients, but clot prevention remains necessary. Robotic assistance and a mini-subvastus approach do not eliminate VTE risk. Bilateral and revision procedures may involve greater physiological stress, anaemia or mobility limitation and require an individual prevention plan rather than copying a routine primary-replacement prescription.
What Patients Should Avoid
Do not remain immobile for prolonged periods without a medical reason, massage a painful swollen calf, stop or double anticoagulants independently, use another patient’s medicine, add aspirin or supplements without advice, or delay assessment of sudden breathlessness. Prevention should be consistent and coordinated rather than improvised.
When Prompt or Emergency Care Is Needed
Seek prompt medical assessment for new calf pain, tenderness, warmth, redness or substantially increasing one-sided swelling. Seek emergency care immediately for sudden breathlessness, chest pain, coughing blood, rapid heartbeat, light-headedness, fainting, collapse or severe unexplained weakness. Do not travel to a routine clinic appointment when pulmonary embolism is suspected.

How long does clot risk last?
Risk is highest during early recovery but continues after discharge while mobility and the postoperative clotting response remain altered.
Does every patient need the same blood thinner?
No. Drug and duration depend on clot risk, bleeding risk, medical conditions and the operation.
Can a clot occur while I am taking medicine?
Yes. Prevention reduces risk but cannot eliminate it completely.
Can a DVT have no symptoms?
Yes. Some DVTs are asymptomatic, which is why risk-based prevention is important.
Should I massage a swollen calf?
No. A painful swollen calf should be medically assessed rather than massaged.
Can normal swelling be mistaken for DVT?
