Knee Pain Without an Injury
Knee pain often begins without a memorable fall, twist or collision. Arthritis, repeated loading, kneecap irritation, tendon or bursal pain, degenerative meniscal change, gout, inflammatory disease, weakness and pain referred from the hip or spine may all develop gradually.

Common Causes
Knee osteoarthritis
Osteoarthritis may cause activity-related pain, stiffness after rest, swelling, reduced walking distance, stair difficulty, deformity or night pain. Symptoms can fluctuate without a new injury. Kneecap, tendon, bursal and meniscal causes
Pain around or behind the kneecap can appear after increased walking, stairs, squats, prolonged sitting or changes in training. Tendon pain may follow repeated stairs, jumping or running; bursitis may follow kneeling or pressure. A degenerative meniscal tear can develop without dramatic trauma and may coexist with arthritis. Crystal, inflammatory, infectious and referred causes
Gout may cause sudden severe pain, redness and swelling. Inflammatory arthritis may cause prolonged morning stiffness and symptoms in several joints. A hot red swollen knee with fever or illness requires urgent assessment because infection must be excluded. Localised tendon pain may worsen with stairs, jumping or resisted movement.
Sudden redness and swelling may suggest gout or infection.
Burning, numbness or radiating pain may suggest a nerve or spine source.
Persistent unexplained night pain, weight loss or systemic illness requires broader assessment.
How It Is Evaluated
Evaluation includes onset, pain location, swelling, morning stiffness, activity triggers, night pain, mechanical symptoms, fever, medicines, medical conditions and functional limitation. Examination assesses gait, alignment, movement, swelling, tenderness, kneecap tracking, strength, ligament stability, hip movement and neurological or circulation signs when required.
Weight-bearing X-rays are useful when arthritis, deformity, fracture or bone disease is suspected. Treatment Without Surgery
Treatment should match the diagnosis and may include temporary modification of provoking activity, therapeutic exercise, quadriceps and hip strengthening, aerobic activity, weight management when appropriate, footwear or walking-aid advice, and medication selected according to medical history and risk. When Are Injections or Surgery Considered?
Injections may be considered only for selected diagnoses and should not be presented as guaranteed cartilage-regrowing treatment. Does knee pain without injury always need an MRI?

Can weakness cause knee pain?
