Hip Pain Locations and What They May Mean

Hip pain location can narrow the list of possible causes, but it does not establish a diagnosis by itself. Pain from inside the hip joint is often felt in the groin or front of the thigh and can be referred to the knee. Pain over the outer hip more often arises from tendons or the bursa, while buttock pain may come from the hip, spine, sacroiliac region, nerves or muscles.

  • Groin or front-of-thigh pain commonly suggests an intra-articular hip source.

  • Outer-hip tenderness commonly suggests greater-trochanteric tendon or bursal pain.

  • Buttock pain, burning, tingling or symptoms below the knee raise concern for spine or nerve involvement.

  • Hip disease can present mainly as thigh or knee pain.

  • Location must be combined with onset, movement, stiffness, gait, examination and selected imaging.

Groin and Front-of-Hip Pain

Deep groin pain is common in hip osteoarthritis, avascular necrosis and other joint disorders. It may worsen with walking, stairs, shoes and socks, entering a car or rotating the leg. Painful restriction of hip rotation supports a joint source, but hernia, abdominal, urological and other non-orthopaedic causes may also require consideration.

Outer-Hip Pain

Pain over the greater trochanter may arise from gluteal tendons, local bursal irritation or altered gait. It can be tender to touch and worse when lying on that side, climbing stairs or standing on one leg. This pattern is different from deep groin pain, although both can coexist.

Buttock, Thigh and Referred Knee Pain

Buttock pain may be referred from the lumbar spine, sacroiliac region or nerves, but hip arthritis can also be felt posteriorly. Hip-joint disease can also be felt in the front of the thigh or around the knee. Hip rotation, gait and groin discomfort should be checked when knee findings do not fully explain symptoms.

How the Pattern Helps

  • Weight-bearing pain may fit arthritis, AVN, stress injury, fracture or mechanical overload.

  • Marked morning stiffness and several painful joints may suggest inflammatory disease.

  • Unrelenting night pain, fever, weight loss or rapid decline needs review.

  • Pain after replacement has a separate differential including infection, fracture, instability, loosening and soft-tissue causes.

Examination and Imaging

Assessment includes gait, hip movement, strength, tenderness, leg length and a spine and neurological examination when indicated. Weight-bearing X-rays commonly assess arthritis, deformity and fracture. MRI is selected for questions such as early AVN, occult fracture, stress injury or soft-tissue disease; it is not required for every pain location.

Urgent Warning Signs

  • Inability to stand or bear weight after a fall, a deformed or shortened limb, a hot painful joint with fever, new weakness or numbness, bladder or bowel loss, chest pain or breathlessness after recent surgery.

Can hip arthritis cause knee pain?

Yes. Hip-joint pain can be referred into the thigh or knee, especially when knee findings do not fully explain symptoms.

Does outer-hip pain mean I need replacement?

Usually not. Outer-hip pain commonly arises from tendons or the bursa and is treated differently.

Can significant hip pain occur with a normal X-ray?

Yes. Early AVN, stress injury, soft-tissue disease and referred pain may not be explained by a standard X-ray.
